Medication Authorization Form

Medication authorization form is required for all over-the-counter and prescription medication that you send with your child. Prescription medication must be in original packaging from the doctor or pharmacy with the child's name printed on the label.

ACH Authorization

ACH authorization form for setting up automatic withdrawals from a checking account for childcare payments. Must be turned in with a voided check.
